چکیده

A 26-YEAR-OLD symptomatic female patient with severe aortic and mitral valve stenoses underwent aortic and mitral valve balloon valvuloplasty in the same setting. Aortic balloon valvuloplasty was first performed retrogradely using a polyethylene 20-mm balloon (Mansfield, USA), and mitral valvuloplasty was performed antegradely through transseptal technique using an Inoue 28-mm balloon (Toray, Japan). After dilation the mean aortic valve gradient decreased from 34 to 19 mm Hg. The gradient across the mitral valve decreased from 20 to 4 mm Hg. The aortic valve area increased from 0.5 to 0.9 cm2 and the mitral from 0.5 to 1.3 cm2.

SHORT TERM CLINICAL OUTCOME OF PERCUTANEOUS BALLOON MITRAL VALVULOPLASTY VERSUS SURGICAL CLOSED MITRAL COMMISSUROTOMY

BaIlon mitral valvuloplasty and closed mitral valve conunissurotomy were performed in 450 and 127 patients, respectively. Also, 254 patients with severe mitral stenosis were enrolled in a prospective randomized trial comparing the two procedures. Immediately after balloon mitral valvuloplasty, mean left atrial pressure was 11.38±3.54 mmHg, transmitral valve gradient was 1.8±2 mmHg, and mit...

Effects of increasing flow rate on aortic stenotic indices: evidence from percutaneous transvenous balloon dilatation of the mitral valve in patients with combined aortic and mitral stenosis.

OBJECTIVES To investigate the effects of transvalvar flow rate on aortic valve resistance and valve area after percutaneous transvenous balloon dilatation of the mitral valve in a homogeneous group of patients with rheumatic heart disease. DESIGN Retrospective analysis of 12 patients with combined aortic and mitral stenosis who had undergone balloon dilatation of the mitral valve over a perio...

High-risk Coronary Artery Bypass Grafting and Mitral Valve Replacement in a HIV Positive Patient

Certain subsets of high-risk mitral valve patients are not suitable candidates for transcatheter therapies. The objective of this report is to present a young patient with combined mitral valve and coronary artery disease to illustrate these challenges. In this report, we present a 47-year-old man with longstanding HIV infection who was referred with severe mitral regurgitation (MR) and profound...

Preventive strategies to fight complications during percutaneous multitrack balloon valvotomy "refining the technique".

BACKGROUND Percutaneous mitral valvuloplasty (PMV) is still the treatment of choice in selected cases of mitral stenosis (MS). Multitrack balloon (MTB) catheter is one of the techniques used for PMV with optimal results. We describe a novel refinement of appropriate balloon sizing and wire placement to reduce mitral regurgitation (MR) and Left ventricular (LV) apical perforation, respectively. ...
